Only a Reflection? Sexual Violence on Screen

Why do so many films and TV programs contain scenes of sexual violence? Is it 'only a reflection' of our society, or do these portrayals serve another purpose? Can sexual violence be depicted realistically, and without exploitation? Is this something to strive for?
